Requesting confidential case reviews
Call Priority Health Behavioral Health to schedule by e-mail or by phone.  When scheduling the review:

• Include only the member's contract number. Do not include the name of the member.
• Do not fax forms for Priority Health employees and/or their families.
• Do not leave messages regarding services for Priority Health employees and/or their families.
