Call Priority Health Behavioral Health to schedule by email or by phone. When scheduling the review:
- Include only the member's contract number. Do not include the name of the member.
- Do not fax forms for Priority Health employees and/or their families.
- Do not leave messages regarding services for Priority Health employees and/or their families.
