Chlamydia urine test tool kit
Follow this process to set up your office to receive results by fax, order test kits, and submit samples for testing to the Kent County Regional Laboratory (regardless of the location of your office).
- It will save you time, and you'll get the results much faster. Also, your office won't have to schedule an unwilling teen for a Pap test.
- If the chlamydia test is positive, the report is filed with the state automatically and a gonorrhea test is performed automatically.
- The patient is guaranteed confidentiality of all health information, since the Health Department is bound by federal HIPAA regulations and Priority Health billing information to members is generic and does not list the chlamydia test.
- Set up your office to receive results by fax:
You will need a dedicated fax line at your office. Complete the Statement of Understanding and write a letter on your office letterhead requesting to become an automatic fax agency. The letter must be signed by a person authorized to make this request. Fax both the statement and letter to:

(Priority Health uses the KCHD to process all chlamydia tests, regardless of your location).

Order test kits:
Use this form to order test kits. The collection tubes have expiration dates, so order no more than the volume of kits you will use in a 3-month period. The cost of the kits is included in the testing fee, so you pay nothing.

- Collect samples:
Follow the instructions on this sheet from the Kent County Regional Laboratory for collecting the samples.

- Send in samples for testing:
- On the test requisition form (below), check the Priority Health Member box at the top and send it to the Kent County Health Department laboratory address regardless of the location of your office, with the sample(s). Do not use lab slips from other labs.

- You may mail up to 5 samples in the USPS-approved triple packaging which is included with the kits. Postage is approx. $1.70. Or, you may drop them off directly at the laboratory, or use a courier (does not require the triple packaging, but a biohazard-safe container/bag/cooler). Specimens have a 30-day limit. They may be refrigerated.
- Results will be faxed to your office in 24-48 hours if you have requested this service (see step 1) or by mail in 3-5 days. Online results are planned for 2009.

If you bill both, they will cancel each other out.
