The Medicaid program pays for health care for individuals and families with low incomes and resources. Priority Health is a Qualified Health Plan administering Medicaid health benefits in several counties in Michigan.

Find out if you're eligible for Medicaid. To get an application:

• Visit your local Michigan Department of Human Services (DHS) office (view a map) or
• Go to www.michigan.gov/dhs
