Completing your HealthbyChoice Rewards actions

This program encourages you to take specific actions by a specific deadline to improve your health. When you participate in HealthbyChoice Rewards, those actions might include:

• Taking the WebMD® HealthQuotient® online health risk appraisal
• Getting a physical exam
• Other actions your employer chooses

The HealthbyChoice rewards you can earn

Your employer decides what rewards to offer when you complete all your actions on time. Check with your company's Human Resources staff for more information.
