HealthbyChoice Incentives^SM is a wellness incentives program linked to your Priority Health medical plan. It encourages you to make healthier lifestyle choices to qualify for lower costs on your health plan.

HealthbyChoice Incentives plan features

• If you meet the requirements for the Choice level, you'll earn a lower deductible and you'll pay less in copays and/or out-of-pocket costs.
  Here's how qualify for the Choice benefit level.
• If you don't qualify for the Choice benefit level, you'll pay for services at the Standard level.
• Your medical benefits don't change. The same medical services will be covered no matter whether you're on the Choice or Standard level. You'll just pay a higher copay for them.
  
• At the beginning of your next plan year, you keep your level - Choice or Standard - for the first 90 days. You can complete the requirements to earn the Choice level again.
• If your plan also covers your husband or wife, they have to meet the requirements each year, too.
