Forgot your username or password?

No problem.

1. On the home page of priorityhealth.com, click Forgot your Username or Password?
2. Enter your contract number from your Priority Health member ID card, your date of birth, and the last four digits of your Social Security number in the boxes.
3. Our system will give you your username.
4. Enter a new password. (We don't have access to your old password, so we can't tell you what it is.)
