If you forget your username or password

No problem.

  1. On the home page of priorityhealth.com, click Forgot your Username or Password?
  2. Enter your contract number from your Priority Health member ID card, your date of birth, and the last four digits of your Social Security number in the boxes.
  3. Our system will give you your username.
  4. Then just enter a new password. (We never provide old passwords, either online or over the phone.)
