We're in Five More Counties for 2008!
We've added five new counties in which you can enroll in our three Medicare Advantage plans - PriorityMedicare Value, PriorityMedicare and PriorityMedicare Plus. (PriorityMedicare Rx is available statewide.) The new counties are:

Enroll Soon for 2008 Coverage
Unless you are newly eligible for Medicare, you may enroll in or disenroll from a plan only during specific times of the year. For 2008 coverage, you must sign up between November 15 and December 31, 2007.

  • You may also come to the walk-in centers at our Grand Rapids and Holland offices, from 8:30 a.m. to 4:30 p.m., Monday through Friday. For directions, call the phone numbers above or visit priorityhealth.com/medicare.
If you are newly eligible for Medicare or you do not have creditable coverage, your premium will be higher if you wait to join after December 31, 2007. You can avoid paying this extra money by deciding to enroll in one of our PriorityMedicare plans now.
Turning 65 soon? You've probably started exploring your Medicare choices. It can be a bit overwhelming at first, but rest assured that Priority Health is here to help make Medicare easier. As a local resource for information on Medicare, we provide a lot of ways for you to learn everything you need to know about your Medicare options. For example:
  • We conduct free informational meetings at area restaurants. (See "Learn More".)
  • We have walk-in centers at our Grand Rapids and Holland offices.
  • We offer an information-packed, easy-to-use website, priorityhealth.com/medicare.
  • Our representatives are available to answer your questions and will be happy to send you additional information.
We'll help you understand the differences between the Original Medicare and Medicare Advantage plans. We can assist you in sorting through Medicare Part A, B, C and D and learning about your options for prescription drug coverage. We'll make it all easy to understand so that you can choose the plan that is best for you.

In addition, Priority Health is contracted with the federal government to provide Medicare plans.1 We make our plans easy to understand and use. You may find that our plans offer you the most value for your money. Our Medicare plans provide the same level of service you have come to expect from Priority Health. A recent independent survey of new PriorityMedicare members found that 95% are satisfied with our products and service.

All three of these Medicare Advantage plans combine prescription drug coverage with medical benefits2 including:
  • Annual physicals and routine health screenings
  • No referrals needed to see a specialist in our network
  • Inpatient hospital stays
  • Nationwide coverage for medical and hospital benefits
  • Worldwide coverage for emergency and urgent care
These three medical plans are available in 19 counties for 2008. (See the "Update" sidebar to the left for the list of counties.) You can enroll if you are entitled to Medicare Part A and enrolled in Part B. You must continue to pay your monthly Part B premium, unless it is otherwise paid for under Medicaid or by another third party. Remember, you can be enrolled in only one Medicare Advantage-Prescription Drug Plan at a time.3

Medicare Part D Prescription Plan


PriorityMedicare Rx is a Medicare prescription drug plan that offers prescription drug coverage only. Its features include:
  • Cost savings - Low copayments for prescription drugs can deliver significant annual savings.
  • Convenience - We have more than 60,000 pharmacies, including most major chains, in our nationwide pharmacy network.
It's important to note that prescription drug copayments are applicable if a drug is covered in the PriorityMedicare formulary (approved drug list) and filled at an in-network pharmacy.

PriorityMedicare Rx is available throughout Michigan to beneficiaries who are entitled to Medicare Part A or enrolled in Part B (you must continue to pay your monthly Part B premium). Even if you don't use many prescription drugs, signing up for a Part D plan now means you will have insurance if your prescription needs change. And remember, you can be enrolled in only one Part D plan at a time.4

Extra Help for People with Limited Income and Resources


Beneficiaries interested in qualifying for extra help with Medicare Prescription Drug Plan costs should call:
  • 1-800-MEDICARE (1-800-633-4227). TTY/TDD users, call 1-877-486-2048, 24 hours a day, seven days a week, or
  • Your State Medicaid Office, or
  • The Social Security Administration at 1-800-772-1213, from 7 a.m. to 7 p.m., Monday through Friday. TTY/TDD users, call 1-800-325-0778.

Help for the Hearing- or Speech-Impaired
In addition to meeting with us, a telecommunication device (TTY) is available to help people who are deaf get additional information or set up a meeting with a sales representative.

  1. Priority Health's contract with Medicare is renewed annually, and the availability of coverage beyond the end of the current contract year is not guaranteed.
  2. Copayments and benefit limits may apply. Members of PriorityMedicare Value, PriorityMedicare and PriorityMedicare Plus must use in-network providers to receive the full plan benefit, except under emergency circumstances. You may choose to see providers who are not in our network, but you will pay 20% coinsurance (or more) and one initial deductible of $500 for all Medicare-covered plan services that are received out-of-network. A member's annual out-of-pocket maximum is limited to $5,000 for PriorityMedicare Value and $2,000 for PriorityMedicare and PriorityMedicare Plus. See Evidence of Coverage for details.
  3. The Medicare Advantage prescription drug benefit offered by PriorityMedicare is available only to members who are enrolled in our plan. If you are already enrolled in a different Medicare Advantage-Prescription Drug Plan, you must receive your drug benefit through your current plan.
  4. If you're already enrolled in a Medicare Advantage (MA) plan, you may not enroll in a separate Prescription Drug Plan unless you are a member of a Private Fee-for-Service MA plan that does not provide Medicare prescription drug coverage, a Medical Savings Account MA plan or an 1876 Cost Plan.
