To make sure you're receiving all the services and screenings you need to stay healthy, our computers scan your claim records and information your doctor enters to find care you may need. This information automatically creates the Personal Health Record and Action List that you see when you're logged in to your Priority Health account online.

Your Personal Health Record shows you:

Preventive care opportunities
For example, if you haven't had a physical exam in 2 years, our computers automatically add a physical exam to your Action List.

• Physical exams
• Immunizations
• Cancer screenings, like Pap tests and colonoscopies
  

Disease management care opportunities
If your records indicate you might have one of these conditions and have not received the follow-up care recommended for it, our computers automatically add that care to an Action List for you.

• High blood pressure
• High cholesterol
• Heart disease
• Asthma
• Diabetes
