HealthbyChoice Incentives plan

HealthbyChoice Incentives^SM is a health plan combined with a wellness program. It encourages you to make healthier lifestyle choices in exchange for the better of two benefit levels (Choice versus Standard.) Both benefit levels offer the same coverage, but the Choice level saves you on copays, deductibles/out-of-pocket costs and coinsurance.

Meeting the requirements

To take advantage of the Choice benefit level, every year you're in the plan you must meet two requirements that reflect your lifestyle choices to determine if you are striving towards and maintaining a healthy lifestyle. (If your spouse is covered under the plan, he or she also must meet the two requirements.)

If you're a new member, you must meet the requirements within 90 days of your "effective date" - the date your coverage begins. If you're already a member, you  must meet the requirements within 90 days of your renewal date.

The two requirements

1. Health risk assessment: Complete an online health risk assessment (HealthQuotient^TM), which is available when you log in to your account here on priorityhealth.com and click on HealthbyChoice. This tool calculates your health risk levels, and provides recommendations for improving your health.

2. Qualification form: Have your health care provider (typically your primary care physician) submit the HealthbyChoice Incentives qualification form which notes your status on three health indicators. Your health care provider can submit the information online or by faxing us the qualification form (99KB PDF).
