SM (Priority Health's health plan designed to work with a health savings account), the way your POS plan works changes slightly from our basic POS plan. Here's what's different.

| Prescriptions |
- What you pay for prescriptions does not go toward your deductible or your out-of-pocket maximum.

- The amount you pay for prescription drugs first goes toward satisfying your deductible.
- Once you meet your deductible, the amount you pay for prescription drugs goes toward your out-of-pocket maximum.
- Ordering through our mail service will not save you copays before you meet your deductible, because you are paying 100% of the cost of your prescription.

| Preventive care |
- Your plan covers preventive care before you meet your deductible, but you pay copays and coinsurance.

- Preventive care services listed in our Preventive Health Care Guidelines are 100% covered when you use an in-network doctor or other health care provider.

| Office visits |
- You pay copays for office visits after you meet your deductible.

- You pay coinsurance for office visits after you meet your deductible.

| Maternity care |
- Routine prenatal care is covered, but you may have copays for certain office visits or tests.
|
- Prenatal and postnatal care are considered preventive care, and are 100% covered before you meet your deductible when you use an in-network doctor or other health care provider.
- Delivery services are not preventive care, so you pay 100% of the cost until you meet your deductible.
- If you've met your deductible, you pay coinsurance for the delivery.
