Checking your HealthbyChoice status

Log in to your account on this website to see your progress in meeting the requirements.

  1. Click the Log in link at the top of any page and enter your username and password. If you don't have an online account yet, create an account now.
  2. Click the View Status button in the HealthbyChoiceSM area at the top of the page.
  3. On your status page, you'll see:
    • A list of your HealthbyChoice plan requirements
    • Next to each requirement, a symbol telling you whether or not you have met it
    • Whether or not your spouse or domestic partner has met the requirements (if required by your plan).
    • Your achievement date (the date you met all Choice level criteria)
    • A reminder of when you need to re-qualify for Choice level (appears after you've been at Choice level for 9 months)
