Checking your HealthbyChoice status
Log in to your account on this website to see your progress in meeting the requirements.
- Click the Log in link at the top of any page and enter your username and password. If you don't have an online account yet, create an account now.
- Click the View Status button in the HealthbyChoiceSM area at the top of the page.
- On your status page, you'll see:
- A list of your HealthbyChoice plan requirements
- Next to each requirement, a symbol telling you whether or not you have met it
- Whether or not your spouse or domestic partner has met the requirements (if required by your plan).
- Your achievement date (the date you met all Choice level criteria)
- A reminder of when you need to re-qualify for Choice level (appears after you've been at Choice level for 9 months)
