Taking this free, online tool is one of your Choice-level requirements. Plus, you'll learn how your health stacks up against others your age and get practical tips for feeling your best. It's good for your health and good for your wallet!
Get started
1. Gather information.
If you can't find everything now, start your appraisal anyway. You can add missing information later.
- Height
- Weight
- Waist measurement
- Blood pressure level
- Cholesterol levels
- Triglyceride level
- Blood sugar level
- Month and year of most recent exams (i.e., physical, vision, dental, prostate, mammogram, flu vaccine)
2. Go to the online appraisal.
You’ll see detailed instructions and a link to the tool. Before you get there, though, you’ll need to login to your online account. If you haven't set up access to your account, you can create a username and password. Have your member ID card ready. Learn how to access your account.
Once you're done
- You’ll see your score (if you don't see your score, it's not complete!), a WebMD® HealthQuotient® summary and your health risks.
- Print the summary for your records so you can compare it to future scores and show it to your doctor.
- Click the "Return to Priority Health" link next to your name at the top of the page to go back to your account.
- Click "Go Now" in the yellow HealthbyChoice area to see other actions that are required by your program.
Updating your health risk appraisal
Follow the steps above for completing your health risk appraisal. Click "Update your HQ now” once you’re on the WebMD® HealthQuotient® page.
Each time you go back and update your answers, your score and recommendations are updated.
