HealthbyChoice Incentives instructions

New to HealthbyChoice Incentives

All new members of a HealthbyChoice IncentivesSM plan, including spouses and covered dependents, start out with the Choice level for the first 90 days after your "effective date" - which is the date your health plan coverage begins. Below are step-by-step instructions for what you (and your spouse or domestic partner, if they're also on your plan) both need to do within these 90 days to stay at the Choice level the remainder of the year.

Renewing members

When your Priority Health health plan year is up and you renew again, you'll stay at whichever level you're on - Choice or Standard - for the first 90 days of the new plan year. During these 90 days, you (and your spouse or domestic partner, if they're also on your plan) must both re-qualify for the lower-cost Choice level. If you don't both qualify, you'll receive the Standard level.

You have 90 days to meet the two requirements

In the 90 days after your "effective date" - the date your coverage begins or your yearly renewal date - you and your spouse or domestic partner, if covered by the plan, must meet both requirements, including the alternate requirement if necessary.

1. Complete the HealthQuotient® online health risk appraisal.

Log in to your account here at priorityhealth.com and click on HealthbyChoice. Complete the WebMD® HealthQuotient®, which is an online health risk appraisal made up of many questions about your health. It will calculate your health risk levels and give you recommendations for improving your health. All your answers are private and will never be shared with your employer without your knowledge and permission.

2. Meet the 3 health indicators.

Your primary health care provider (usually a doctor) has to complete the HealthbyChoice Incentives qualification form. It shows your status on three health indicators, below.

  • He or she can send in the information online or by faxing us the form. Print a HealthbyChoice Incentives qualification form (298KB PDF).
  • You have to see your health care provider and get any tests you may need early enough so they can send us the results within your 90-day qualification window.
  • You will typically pay an office visit copay for meeting with your doctor or other health care provider to measure the 3 health indicators. Also, the costs of any additional tests may apply to your deductible.

About the health indicators

To qualify for the Choice level, your doctor or other primary health care provider must tell us:

  • You don't use tobacco of any kind.
  • Your blood pressure is under 140/90.
  • Your body mass index (BMI) is under 30.

If you don't meet the 3 health indicators

You can still qualify for the Choice level if, in the same 90-day window, you do all three of the things listed below, AND your doctor or other primary health care provider lets us know using the HealthbyChoice Incentives qualification form (68KB PDF). You must:

  • Complete a fasting cholesterol test (requires lab testing).
  • Complete a fasting blood surgar test (requires lab testing).
  • Agree to follow your doctor's treatment plan.

Using existing health data and tests

If your doctor or other primary health care provider has already evaluated your health indicators within six months of your effective date of coverage, you can ask that their office send the information to Priority Health so you don't have to make an office visit.

If you need to complete lab work to meet the alternative to Requirement 2, your doctor can submit test results from up to six months prior to your effective date of coverage.

FAQs

Q: Why were the three specific health indicators selected?
They are health behaviors you can control that are linked to serious illnesses and other health issues. The U.S. Centers for Disease Control and Prevention estimate that 50% of today's health care costs are due to health risks that we can change through making changes in our habits, such as improving our diet and getting more exercise.

Q: Can I use results from a biometric screening I had at work?
No. The health indicators must be measured and reported by your doctor, and submitted on the qualification form.

Q: Why are spouses required to participate?
Studies show that couples working together toward better health are more likely to be successful.

Q: Why do I have to meet the requirements again every year?
By meeting the requirements every year, you'll be more aware of your health and the potential health risks that you can correct before they become more serious. And that's the whole purpose of the HealthbyChoice Incentives program.


Get access to your account online

  1. Go to the Create account tool. There's a link to it below, following these instructions.
  2. Fill in your:
    • Contract number from your Priority Health ID card (each family member has a different contract number).
    • Date of birth
    • The last 4 digits of your Social Security number OR
      the last 5 characters, not counting the hyphen, of your Medicare Claim number
  3. Enter a username and password. Choose ones you will remember, since you will use them each time you want to return to your account. Your password must include at least one letter and at least one number.
  4. Pick a security question and answer it.
  5. Click Go to Your Member Center to see all your account information.

Create your account now.

ALWAYS LOG OUT when you are finished visiting your personal online account to protect your personal health information. Simply click Logout in the upper right corner.

If you forget your username or password

If you've forgotten either your username, your password or both, click Forgot username or password on the home page of this website and enter your contract number from your Priority Health member ID card and other information. You will be given your username and asked to create a new password. Then you'll be able to log in to your account.


Checking your HealthbyChoice Incentives status

You can go to your online account here at priorityhealth.com to check your progress in meeting the requirements. You'll also be able to tell if your spouse has met his or her requirements.

  • Log into your online account with your username and password.
  • Find the HealthbyChoice area at the top of your personal page.
  • Click the View Status button.
