Requalifying for Choice benefits

To keep Choice level benefits, you and your spouse or domestic partner (if covered) both must meet the requirements again each year for as long as you're enrolled in your group HealthbyChoice AchievementsSM plan.

When to requalify for Choice level

Your "achievement date" is the month and day when you first meet all the requirements for Choice level. It could be the date you complete the HealthQuotient® health risk appraisal or the date your qualification form shows that you met all five health criteria, whichever is later.

  • We'll send you a letter 90 days before your achievement date anniversary to remind you to re-qualify for Choice.
  • We must receive your qualification form by your achievement date to keep you at Choice level.

How to requalify

Keep track of your achievement date, and make sure you plan a visit to your doctor within the 6-month period leading up to the date so your doctor has recent health data for your qualification form.

Before your one-year achievement date anniversary, you (and your spouse or domestic partner, if they're on your plan) must:

  1. Log in and update your HealthQuotient® health risk appraisal, and
  2. Have your doctor send in new measurements for you showing that you still meet all the health criteria.

If you don't meet the requirements in time

If you don't re-qualify by the one-year anniversary of your achievement date, then you will go back to Standard level costs.

You may try to re-qualify again at any time you think you can meet the health criteria.
