Q: Why don't I see a
HealthbyChoice box when I'm logged in?
A: Only members in employer groups who have elected HealthbyChoice will see it.

If you've just enrolled in your employer's HealthbyChoice plan, you will get access to HealthbyChoice after we receive your enrollment information and it is entered into our system. If you have been enrolled in the HealthbyChoice plan longer than a week and still cannot log in, contact Customer Service.


Q: I switched to Priority Health from another insurance plan and have had a physical exam in the last two years. Do I need to have another one?
A:  No. Contact Priority Health Customer Service.  They will ask you for the date of your last physical exam and proof that you had it, such as an Explanation of Benefits letter, bill from the doctor's office, or a letter from the doctor's office. They can determine if you had the correct procedures to count as a physical exam, and then they'll update your actions status.

Q: I missed the due date for completing the WebMD HealthQuotient. Can I still take it?
A: Yes, you can take or update the health risk appraisal at any time.  But to earn your employer's rewards, you need to take HealthQuotient by the deadline.

Q: I had a physical exam in the last two years. Why isn't it showing up on the Rewards status page?
A: Physical exams are entered in the system when doctors bill Priority Health for them. It may take a few weeks to receive the information from your physician's office. If you log in to your online account and click View Claims, you can see if we have received the claim from your doctor's office. If it's there but your Rewards status page still is not showing that you completed your physical exam, contact Customer Service. They can determine if you had the correct procedures to count as a physical exam and update your actions.


Q: The My Rewards page says I should be involved in a disease management program, but I don't believe this is correct.  What can I do?   
A:  Contact Customer Service. They have a process for reviewing the case and updating this action if it doesn't apply to you.


Q: I logged in in the past but I can't remember my username or my password. How can I get my username and password?
A: On the priorityhealth.com home page, click Forgot username or password? The system will ask you for your Priority Health contract number, and then tell you your username and ask you to create a new password.


Q: The system says my account has been deactivated. How do I reactivate my account?   
A: On the priorityhealth.com home page, click on Forgot username or password? and enter your Priority Health contract number. The system will walk you through reactivating your account.


Q: I have HealthbyChoice Rewards requirements due for two different employers. How do I make sure that complete all the actions for both plans?   
A: You can see what actions you need to take for each employer separately. After you log in and click the Go now button under HealthbyChoice, the next screen you see will ask you to choose one of your two Priority Health contract ID numbers. Then you'll click Go to get to WebMD; then click Rewards status to see that employer's actions and deadlines.  After you're done with one employer's requirements, you'll have to log out. Then, go back to your Priority Health account, and then choose the other employer's plan. You'll have to complete some actions, like the HealthQuotient® health risk appraisal, twice, once for each employer, to receive credit from both employers.
