Q. What is a HIPAA form, and why do I need to have one on file?
A HIPAA (Health Insurance Portability and Accountability Act) form is needed to release personal health information to someone other than you. If you're unable to act on your own behalf, it might be helpful to let another person, such as a spouse, have access to your health plan information.
This can only happen if we have a signed HIPAA form on file.
