Carson City Hospital provides diabetes education through the Diabetes Outpatient Program of Montcalm County. A nurse educator teaches signs, symptoms, and treatment of hyperglycemia and hypoglycemia conditions, glucose monitoring, physical changes diabetes may cause, drugs, insulin, and other risk factors. There are also one-on-one visits available.

Children have fun while learning how to better manage their diabetes. Physicians, dietitians and other health professionals staff the camp. (Priority Health offers limited scholarships.)

The nurse facilitates group educational classes on diet, exercise, and behavior change. The dietitian reviews meal plans, signs of high and low blood sugars, and other important topics for people with diabetes. Follow up is on an individual basis.

Holland Community Hospital offers one-on-one diabetes education with a Registered Nurse and Dietitian. The hospital also offers a diabetes support group, group classes, and a comprehensive class twice a month.

Metropolitan Hospital in Grand Rapids offers Outpatient Diabetes Self-Management Education classes for people ages 18 and over with Type 1, Type 2 and gestational diabetes. Education focuses on learning about diabetes: treatment, self-care, medication, and risk factors involved. A dietitian and nurse will work together to educate you on lifestyle changes and goals to be met. Each participant will receive an individualized meal plan. (Requires physician referral.) State- certified and recognized by the American Diabetes Association. Translator services also available.

The goal of the Initiative is to reduce the incidence of diabetes and improve the care of people with diabetes in this region. Their website has resources for people at risk for diabetes, newly diagnosed with diabetes, and people who have had diabetes for a long time. In addition, there are resource for health professionals caring for people with diabetes.

North Ottawa Community Health offers a diabetes education program which consists of a 60-minute individual assessment and test that identifies each person's risk factors. After the assessment, there are three three-hour classes taught by a dietitian and nurse. These classes cover medications, different types of diabetes, self monitoring, risk factor reduction, nutrition and diet, stress management, goal setting, exercise/activity, and lifestyle changes. Follow- up after the program ends checks on the status of personal goals.

Dietitians, Registered Nurses, diabetes educators, and podiatrists are available for classes on meal planning, diabetes management during illness, blood sugar monitoring, community resources, and short and long term complications. There are also advanced topics such as cooking tips, eating out, and traveling tips. Services are provided through the Peter M. Wege Center for Health & Learning.

Group classes and one-on-one appointments are held daytime and evenings. Class types include a comprehensive class for type 2 diabetes (covers all main areas of diabetes self-management) and an intensive management class. A physician referral is needed.

The goal is to promote the awareness of proper care needed by people with diabetes, such as a physical exam, dilated eye exam, foot checks, and an HbA1c test.
