Have you learned that you or your child has asthma? Do you just want more information about how to prevent asthma, or how to care for someone who has it? Members of the Priority Health Asthma Program receive postcards from our Health Management team, reminding them of important ways to stay their healthiest.

Talk with a nurse case manager about your asthma

If you need extra help - perhaps because you are dealing with additional health risks - a case manager will help you:

  • Manage your asthma, triggers or other asthma risks
  • Send you educational materials
  • Work with your doctor if necessary
  • Answer questions and offer support
