Before you start
1. Gather this information.
If you can't find everything now, start your appraisal anyway. You can add missing information later.
- Height
- Weight
- Waist measurement
- Blood pressure level
- LDL/HDL/total cholesterol levels
- Triglyceride level
- Blood sugar level
- Month and year of most recent exams (i.e., physical, vision, dental, prostate, mammogram, flu vaccine)
2. Go to your Priority Health account online.
If you haven't set up access to your Priority Health account online, you'll need to create a username and password to log in here at priorityhealth.com. Have your member ID card ready.

Tips for taking your health risk appraisal
- The first time you go to the WebMD® site to take the HealthQuotient® health risk appraisal, check the box that says you have read their Terms & Conditions and Privacy Policy. Then click Submit.

Save often! After 20 inactive minutes, your connection to WebMD® will close, and you'll have to start from the last page you saved.

Be sure you see your HQ score at the end. If you don't, it's not complete.
- Look for a Print summary link so you can show your results to your doctor, or compare them with your scores in the future.
