Education programs from Priority Health

What are health management programs?

Priority Health health management programs help members of our medical plans learn to manage ongoing health conditions such as asthma, heart disease, depression, smoking, and pregnancy. They include information we mail to you and other kinds of help.

How do I sign up for a program?

We'll enroll you in a program automatically with a referral from:

  • A doctor or hospital that has billed us for medications you've used
  • Your primary care physician
  • You. Contact Priority Health Customer Service, and a case manager will call you within 3 days to discuss your condition.

What's included in the programs?

Program participants receive:

  • A welcome packet
  • Reminder mailings for services due
  • Educational materials
  • A call from a nurse case manager if you have a significant health risk

You will also have access to online condition centers (available once you log in to your personal Priority Health account).

Can I participate in more than one program?

Yes. If you need help managing more than one health condition, you can be signed up for more than one program.

What if I'm referred to a program and I don't want to participate?

We hope that you'll find these programs helpful. However, if you prefer not to participate, contact Priority Health Customer Service to disenroll.
