Dealing with complex or serious health issues? Let us help.

Priority Health has an in-house team of more than 70 nurses, doctors, pharmacists and certified case managers to make sure you get the care you need, especially when you have more than one disease or condition. What we do is called "complex case management."

When can you get case management help

You may get a call from one of our nurse case managers if:
  • You ask for help and advice; email us and one of our nurse case managers will call you back. Request a call.
  • Your doctor refers you to us for help
  • Our records show that you have more than one serious health condition, such as asthma, diabetes, high blood pressure or depression
  • Your medical costs are mounting up (we typically check in with you at much lower cost levels than other health insurance companies, because the earlier you get help, the better)
  • You go to an emergency room or are admitted to a hospital
  • You are taking many medications

What's included in complex case management

Case management services are optional, but they can be very helpful. What you get depends on what help you need. Your personal services may include:
  • Educational information about your specific conditions, including links to online tools, condition support groups and more
  • "Ask a nurse case manager" service
  • Pharmacy management, where our in-house pharmacists check to make sure you're taking the best medications for your conditions
  • Care coordination between all your doctors and other medical services to help you navigate the health care system
