Surgery education online
Priority Health is linking our plan members to a free online surgery education program. You click through it at your own pace, stopping and coming back whenever you like. It was created by doctors, surgeons, nurses and other health care experts to make sure you know what to expect from surgery and are making smart choices to get the best results for you. And it's completely confidential.
Using videos, Q&As and more, you'll learn about:
- Getting the right diagnosis
- Finding the best doctor
- Making the right treatment decision
- Choosing a hospital
- Preparing for surgery
- Recovering at home
Earn a reward just for trying it
Use the program and you'll receive a code good for $25 ($10 for Medicare members) to spend in an online rewards store powered by Amazon. Choose from many health-related items - yoga mats, DVDs, hand weights, health magazines, etc. Want to preview what items are available? Go to the rewards store now.
If you're a member of a Priority Health PPO, POS or HMO plan, you can choose a $10 Meijer gift card instead of a reward code.
This offer replaces all previous offers and is subject to change at any time. This offer is limited to one reward per year for each eligible member, but you can use Welvie as often as you'd like.

Who is eligible for this surgery education program
- Members of PriorityHMOSM, PriorityPOSSM, PriorityPPOSM, Priority Health Medicare plans, and MyPrioritySM individual and family plans can use the program anytime; they're eligible to earn one reward per year.
- Members of Medicaid or MIChild through Priority Health or PriorityASO or PriorityEPO employer-funded plans cannot use the program at this time.
How to use the program and earn your reward

You must set up online access to your Priority Health account to use the program. If you haven't already created a username and password on this website, set it up now. You'll need the ID number on your Priority Health member ID card and the last 4 numbers of your Social Security or Medicare number.

Complete steps 1 - 3 of the surgery decision support program. It's available on a website called Welvie.
Go to the Welvie website now.

Complete a short feedback survey that will be emailed to you within 48 hours of when you finish step 3. Verify your email address to receive your reward code and instructions on how to redeem your reward.
