Before you enroll

What you need to enroll


Before you enroll in a PriorityMedicareSM plan online, by phone or by mail, have the following information available:
  • Your Medicare card, if you are currently enrolled in a plan
  • Your ID and Group number, if you currently have additional prescription drug insurance
  • If you're currently in a long-term care facility, the number and address of that facility
  • Your Medicaid number, if you are enrolled in Medicaid
  • The name of your primary care provider, clinic or hospital. To be sure your provider is a member of the PriorityMedicare network, use our online Find a Doctor tool.

  • If you were referred by an insurance agent, the agent's name and number.
