Enrollment for 2012 begins on Oct. 15

You may only enroll in a 2012 plan early if you are just now turning 65 or if you qualify for a special enrollment period because:

  • You've lost your current coverage
  • You're moving outside of a service area
  • Or other circumstances apply.

To enroll early:

You will need to complete these forms and print them, then mail them to us at the address on the forms.

You can also choose a 2011 plan (until the end of November).

If you have questions, call toll-free 888 389-6676, 8 a.m. to 8 p.m., 7 days a week. TTY users should call 711.

Get a reminder when it's time to enroll

We'll phone you or email you in mid-October, whichever you prefer.
