Choice level requirements not met

You and/or your spouse didn't meet the requirements for the Choice level of benefits. 

You are now at the Standard level of benefits: 

  • The health care services your plan covers do not change
  • Your copayments, deductibles, out-of-pocket costs and coinsurance will be higher
