Priority Health has not received a qualification form from your doctor.

Once we receive the qualification form and learn that you meet all five health criteria, you'll be awarded Choice level. Choice level begins on the day you met the criteria (your "achievement date"), and Priority Health will refund you the difference between Standard and Choice level for any payments you may have made after your achievement date.
