  1. You are on any medication frequently prescribed for asthma; or
  2. You have been diagnosed with asthma or a related condition.
• For cardiovascular disease, Priority Health checks if:
  1. You are on any medication frequently prescribed for high blood pressure, high cholesterol, or other heart or blood condition; or
  2. You have been diagnosed with those conditions, including hardening of the arteries or heart attack risk.
• For diabetes, Priority Health checks if:
  1. You are on any medication frequently prescribed for diabetes or blood sugar control; or
  2. You have been diagnosed with diabetes.
• For depression, Priority Health checks if:
  1. You have been diagnosed with depression during the past three months.
