When you join a HealthbyChoice Incentives^SM plan or renew for another year, you have 90 days to qualify for, or to re-qualify for, the money-saving Choice level of benefits. It's up to you to make sure your doctor or other health care provider fills out and submits your qualification form, and to get any lab tests and treatment you may need, by the 90-day deadline.

If you don't complete the requirements by the deadline shown, you will automatically be assigned the Standard level of benefits for the rest of the plan year.
