The name of your plan, "PriorityHMO^SM" or "PriorityMedicare Value^SM" for example, is printed on your Priority Health member ID card.

Your "network" is all the doctors and other health care providers who have signed a contract with Priority Health to treat members of your plan.

Self-funded plans
If your employer pays for your medical costs instead of Priority Health, you will see "Self-Funded" or "SF" on your card. Priority Health only does the administration of your plan.
