Family alternate deductible

When you have a Point-of-Service plan, covered services are paid at either the Preferred level or the Alternate level, depending upon whether a provider is in-network or not. See your coverage documents for details.

Your Family Alternate Deductible is the total cost of Alternate-level services for all individuals on your policy that you must pay before Alternate-level plan benefits begin to pay.

• Only Alternate-level services covered by your policy apply to the Alternate-level deductible.
• This deductible must be met each plan year.
• Once you have met your family Alternate-level deductible, you don't have to meet everyone's individual Alternate-level deductible.
