If you receive this error message, the four numbers you entered do not match the same person as the Birth Date and/or Priority Health Contract Number you entered.
Note: All three numbers - your contract number, birth date, and last 4 digits of your Social Security number - must match what we have on file for you, not for any other family member who uses the same ID card. This is how we verify that you have a right to see your personal health information.
If you are a new member, you may not yet have been added to the Priority Health database. Wait several days before you try to open a user account.
