FIELD TIPS
Enter the three-digit area code in the first Phone Number field. Enter the three-digit local exchange in the second field, and the four-digit suffix in the third field.
Extensions: If you have a specific extension you can enter this number in the fourth box, but it is not required.
ERROR MESSAGES
A Phone Number is required.
If you received this error message, one or more of the first three Phone Number fields was left blank. We cannot create your User Account without a phone number.
The Phone Number entered is either incomplete or formatted incorrectly.
If you received this error message, please re-enter your entire phone number.
