FIELD TIPS
Enter the Priority Health contract number shown on your card after your
name. If you don't have a card, find your contract number on the documents
your employer or Priority Health sent you.
- Enter it exactly as it is shown, including the hyphen and last two numbers if you see them.
- For most members, it should look like this: 987654321-00.
ERROR MESSAGES
Contract Number is required.
We cannot give you access to your records without verifying your
contract number.
Contract Number is incomplete or formatted
incorrectly.
Enter your contract number exactly as it is shown on your ID card
after your name or on the documents given to you.
We can't verify your Priority Health membership
from the information given.
If you receive this error message, check that your Priority Health
Contract Number field was entered correctly, including the hyphen and
last two numbers if you have them.
Note: Every family member has a different contract number. It appears after your name on your Member ID card.
The contract number and date of birth you enter must match the information we have for you in the Priority Health database before we can create an account for you. This helps us protect the privacy of your health information.
If you are a new member, you may not yet have been added to the Priority Health database. Wait several days before you try to open a user account.
