Vendor Name is the name of your practice or facility. Enter the Vendor Name exactly as it appears on your weekly Remittance Advice listing from Priority Health.

ERROR MESSAGES

A Vendor Name is required.
If you received this error message, the Vendor Name field was left blank. We cannot give you online access to your records without your vendor name.
