Year entered is not valid.
Please enter your birth date this way:
- Use the drop-down menu in the first box to select the month you were born.
- Use the drop-down menu in the second box to select what day you were born.
- Type all four numbers of the year in which you were born in the last box, like this: 1968
We cannot open a User Account for you without your birth date.
We can't verify your Priority Health membership
from the information given.
If you receive this error message, the birth date you entered
does not match the contract number you entered on the previous
page.
Note: Enter your own personal contract number, birth date, and Social
Security number. All three numbers must match one individual.
This helps us protect the privacy of health information.
If you are a new member, you may not yet have been added to the
Priority Health database. Wait several days before you try to open a
user account.
