Coverage documents include:
If you have specific questions about your coverage call Customer Service at the number on the back of your Priority Health ID card.
Coverage documents include:
- The Certificate of Coverage, Evidence of Coverage, Summary Plan Description or insurance policy, depending on your plan;
- Any riders or addendums (additions, changes or deletions) chosen by your employer;
- The Summary of Benefits, and
- A directory of health care providers who participate in the plan.
