What are coverage documents?

Coverage documents include:
  • The Certificate of Coverage, Evidence of Coverage, Summary Plan Description or insurance policy, depending on your plan;
  • Any riders or addendums (additions, changes or deletions) chosen by your employer;
  • The Summary of Benefits, and
  • A directory of health care providers who participate in the plan.
