The WebMD HealthQuotient

Want to know more about what health risks you may be facing? The HealthQuotient® health risk appraisal created by WebMD asks questions about your family, your lifestyle and your health history to create a free health risk report for you. It's private; your employer will never see your answers without your knowledge and permission.

We recommend that you print these instructions to use for reference. You can also call Customer Service for help.

Before you start

Create your online account. You'll need a username and password to log in here at priorityhealth.com.

Gather some information so it's at your fingertips when the HealthQuotient asks for it. You may want to call your doctor or other health care provider to get this information. If you don't have it today, no problem. You can always update your HealthQuotient later.
  • Height
  • Weight
  • Waist measurement
  • Blood pressure level (and date taken)
  • Cholesterol levels (LDL/HDL/total, and date taken)
  • Triglyceride level (and date taken)
  • Blood sugar (and date taken)
  • Month and year of most recent exams (i.e., physical, vision, dental, prostate, mammogram, flu vaccine)

Tips for taking your health risk appraisal

  1. The first time you go to the HealthQuotient, you need to check the box that says you have read the WebMD Terms & Conditions and Privacy Policy. Then click Submit.
  2. Answer each question as accurately as possible.
  3. Once complete, you will see your score, a summary, and your health risks. BE SURE YOU SEE YOUR SCORE, or your HealthQuotient is not complete.
  4. Look for a Print summary link so you can show your results to your doctor, or compare them with your scores in the future.
