Members of Priority Health's HealthyEncounters^SM Diabetes Program receive postcards from our Health Management team, reminding them of important ways to stay their healthiest.

Talk with a nurse case manager

If you need extra help - perhaps because you are dealing with additional health risks - a case manager will help you:

• Manage your diabetes, eye disease, kidney disease or other diabetes risks
• Send you educational materials
• Work with your doctor if necessary
  
• Answer questions and offer support
