It focuses on setting goals, education and individual exercise programs. Phase 1: Inpatient rehab, which gives an introduction to cardiac disease for the patient and his/her family. Phase 2: Outpatient rehab provides education for risk factors and lifestyle changes. Dietitians and social workers are available. Includes monitored exercises. Phase 3: Focuses on maintenance. Provides continued education, risk factor assessment and lifestyle modification ideas. Education on relaxation methods, correct exercise, heart facts (anatomy and physiology), heart disease, risk factors and stress management. Monthly topics address ideas for better control of heart failure, such as a low-sodium diet, exercise, medications, symptom recognition, and more. Participants are encouraged to choose a partner throughout rehab and will enjoy an individualized program of exercises. There is no fee for being a partner to a participant. Educational videos and classes are available on nutrition and on cholesterol and lipid levels. Social workers offer classes in stress management.
