Hackley Hospital offers a Cardiac Rehabilitation Program. It  focuses on setting goals, education and individual exercise programs.  Phase 1: Inpatient rehab, which gives an introduction to cardiac disease for the patient and his/her family.  Phase 2: Outpatient rehab provides education for risk factors and lifestyle changes. Dietitians and social workers are available. Includes monitored exercises.  Phase 3: Focuses on maintenance. Provides continued education, risk factor assessment and lifestyle modification ideas.  Pulmonary rehabilitation is also available.

Holland Community Hospital offers monitored exercise with a physical therapist and health assessment by a nurse. Education on relaxation methods, correct exercise, heart facts (anatomy and physiology), heart disease, risk factors and stress management. One-on-one counseling is available.

Holland Cheer Heart Failure Support Group
For individuals who have heart muscle that doesn't work as well as it used to, often leading to what is called "heart failure." Monthly topics address ideas for better control of heart failure, such as a low-sodium diet, exercise, medications, symptom recognition, and more. Meets the second Tuesday of the month from 3 to 4 p.m. Call for registration and locations.

Metropolitan Hospital offers a heart rehabilitation program that begins with an individual evaluation. Participants are encouraged to choose a partner throughout rehab and will enjoy an individualized program of exercises. There is no fee for being a partner to a participant. Educational videos and classes are available on nutrition and on cholesterol and lipid levels. Social workers offer classes in stress management. Support groups are available.

Phase 1: Inpatient rehabilitation.  Phase 2: Outpatient rehabilitation includes up to 12 weeks of a monitored exercise program.  Phase 3: Includes supervised outpatient exercise for up to one year.

Spectrum Health: Preventive Cardiology and Rehabilitation offers education to those who are in an extended cardiac rehabilitation program and also through an all-day class called "Risky Business." The Blodgett Campus class targets risk factors and brings in exercise physiologists, pharmacists, and other experts to aid in the education.

Throughout the cardiac rehabilitation program, education is provided on nutrition, stress, cholesterol levels, heart disease, exercise, strength training, and risk factors. Referrals to dietitians or to specialists in mind, body and spirit (complementary therapy) are also provided.

Holland Hospital offers the CHEER Heart Failure Program to individuals who have been diagnosed with congestive heart failure. This program provides free individualized educational sessions that can help clients manage their heart failure. CHEER also supports clients in the lifestyle changes that must be made to better control heart failure. Call to find out if you qualify for this free program.
