Are you, or is someone close to you, dealing with cardiovascular disease? Members of Priority Health's HealthyEncounters^SM Cardiovascular Program receive postcards from our Health Management team, reminding them of important ways to stay heart-healthy. You can learn about:

• High blood pressure (hypertension)
• High cholesterol
• Congestive heart failure
• Heart attack prevention, recovery & rehabilitation

Talk with a nurse case manager
If you need extra help - perhaps because you are dealing with additional health risks - a Priority Health case manager can help you:

• Manage high cholesterol, high blood pressure, or other cardiovascular risks
• Send you educational materials
• Work with your doctor if necessary
• Answer questions and offer input
