How to earn your Meijer gift card

1. You need to be logged in to your member account to use this online surgery decision-support program. If you haven't already created an account at this website, click "Register Now" link at the top right of this page. You'll need the ID number on your Priority Health member ID card to create your Priority Health account. If you already have access to your account online, just log in with your username and password.
2. Complete steps 1 - 3 of the surgery decision support program on the Welvie website.
3. Fill in the short feedback survey and verify your e-mail address. If you're eligible, you'll receive your gift card in about 90 days.
