How to earn your Meijer gift card

1. You need to have an online member account on priorityhealth.com to use this program. If you haven't already created an account at this website, register it now. You'll need the ID number on your Priority Health member ID card.
   Register your member account now.
   Go to the surgery decision support website now.
2. Complete steps 1 - 3 of the surgery decision support program on the Welvie website.
3. Complete a short feedback survey and verify your e-mail address to receive your gift card. If you're eligible, you'll receive your gift card in 60 to 90 days.
