HealthbyChoice Rewards - Basic & Intermediate

Lower your health care costs and improve your employees' health by adding the HealthbyChoice Rewards wellness program to your health plan. HealthbyChoice Rewards encourages healthier living by rewarding employees for achieving specific health goals. They become more aware of their health, while you save money. It's a win-win situation.

How HealthbyChoice Rewards works

  • For groups with 51 or more employees
  • Can be added to any of our core health plans (PriorityHMOSM, PriorityPPOSM, PriorityPOSSM)
  • We help you identify "actions," tasks that your employees will complete by a certain deadline to improve their health and earn their rewards.
  • Specific actions built into every HealthbyChoice Rewards program include:
    • Completing the WebMD® HealthQuotient® online health risk survey and setting a personal health goal, such as quitting smoking or getting more exercise
    • Getting a physical exam at least once every two years
    • Seeing a physician at least once a year for chronic conditions such as asthma, cardiovascular disease or diabetes (if applicable)
  • To participate, each employee needs to create an online member account. On the personal account page that welcomes each participant to his or her own account, they'll see a HealthbyChoice "View Status" button; when they click there, they'll see the rewardable actions they need to complete and the deadlines by which the actions are due.
