How HealthbyChoice Rewards works
- For groups with 51 or more employees
- Can be added to any of our core health plans (PriorityHMOSM, PriorityPPOSM, PriorityPOSSM)
- We help you identify "actions," tasks that your employees will complete by a certain deadline to improve their health and earn their rewards.
- Specific actions built into every HealthbyChoice Rewards program include:
- Completing the WebMD® HealthQuotient® online health risk survey and setting a personal health goal, such as quitting smoking or getting more exercise
- Getting a physical exam at least once every two years
- Seeing a physician at least once a year for chronic conditions such as asthma, cardiovascular disease or diabetes (if applicable)
- To participate, each employee needs to create an online member account. On the personal account page that welcomes each participant to his or her own account, they'll see a HealthbyChoice "View Status" button; when they click there, they'll see the rewardable actions they need to complete and the deadlines by which the actions are due.
