How HealthbyChoice Rewards works
- Can be added to any of our core health plans (PriorityHMOSM, PriorityPPOSM, PriorityPOSSM)
- We work with you to create goals (also called rewardable actions) and related rewards, such as saving on health care premiums, cash bonuses, etc.
- Specific rewardable actions can include:
- Taking the WebMD® HealthQuotient® health risk survey
- Setting a personal health goal, such as quitting smoking or getting more exercise
- Getting a physical exam at least once every two years
or see a physician at least once a year for chronic conditions such as asthma, cardiovascular disease or diabetes (if applicable)
- To participate, each employee needs to create an online member account. On the personal account page that welcomes each participant to his or her own account, they'll see a HealthbyChoice "View Status" button; when they click there, they'll see the rewardable actions they need to complete and the deadlines by which the actions are due.
