Setting the rewards

Identifying the participants 

You decide whether your plan focuses on employees only or employees and their spouses. If you choose employees and spouses, remember both of them must meet the criteria to receive rewards.

Defining the parameters

This is where you choose how participants qualify and are rewarded.

Decide:

  • To reward employees for participation and progress or for outcomes only
  • If participants need to complete the online health risk appraisal in 90 days or a different timeframe

The criteria

Participants will need to complete an online health risk appraisal and have their doctor document that they meet these 5 key wellness criteria:

Wellness criteria* Qualification requirements
Tobacco use Non-tobacco user
Body mass index (BMI) Below 30
Blood pressure Below 140/90
Cholesterol LDL below 160
Blood sugar Normal or HbA1c below 8 for those with diabetes

Online health risk appraisal

Our online health risk appraisal is a fast and easy way for your employees to gauge their health. In addition:

  • Employees can easily identify health issues they need to work on and receive recommendations for improving their health.
  • Employees can update their health risk appraisal any time to track their progress toward health goals and wellness criteria.
  • You will never see an individual employee's results.

All employees must create an online account to get to online health risk appraisal.

Your report

Thirty days after the qualification deadline, you'll receive a report that:

  • Identifies all participants
  • Indicates whether or not they completed their health risk appraisal
  • Shows their status for each wellness qualification requirement.

Those who can't meet the requirements due to a medical condition (based on their doctor's determination) will automatically be enrolled in our case and disease management program, giving them the support they need to get and stay their healthiest. Learn more about case and disease management.

Determining the timing

Choose when to reward employees who've met their requirements:

  • Yearly
  • Quarterly
  • Monthly
