Medicare health navigators help seniors plot a healthy course
We're proud to introduce "health navigator", Priority Health's newest service team that helps our Medicare enrollees navigate the often confusing health care system. A "health navigator" is a personal health coach who removes barriers and helps Medicare enrollees better understand their health care coverage and available resources. Services include:
- Working across the health plan and care network to ensure members receive and understand their care options.
- Guiding members through Priority Health networks, medical resources, policies, procedures, etc.
- Acting proactively on members' behalf with complex benefits, gaps in care, and prior authorization processes.
- Facilitating referrals to outpatient case management services.
The health navigator first performs an assessment welcome call to identify potential case- or disease-management candidates early. The welcome call is also used to educate enrollees about how to use the self service web resources on priorityhealth.com.
This program is consistent with Priority Health's commitment to high touch service and helping ease Medicare enrollees into their new plan. Whether members haves an administrative or clinical issue, they will have a single point of contact within Priority Health.
