Get Meijer gift card reward for trying a new surgery decision-support program

Beginning Oct. 1, members enrolled in Priority Health fully funded and Medicare plans will be able to access a new online decision-support program powered by Welvie^SM. The program can improve health outcomes and lower costs by helping your employees and their dependents become more informed when making decisions about surgery.

To encourage our members to try it out, we're offering a Meijer gift card reward to every eligible member who completes steps 1-3 of the 6-step program and a short feedback survey. Eligible members include your employees and their dependents - they don't even need to be considering surgery to get the reward. We're offering the reward because want all of our members to get to know the program now. Once they've experienced it, we're confident they'll want to use the Welvie program if they face a surgery decision in the future.

We have an exclusive agreement for this program in Michigan. It's just another way we're providing extra value by encouraging your employees and their dependents to become more active in decisions about their health and care. That can mean fewer absences and increased productivity for you.

It's friendly and easy to use

Welvie uses an interactive approach to walk members through the surgical journey from diagnosis to recovery. Members answer questions and view video clips to learn treatment options and questions to ask their doctors about their care. Founded by surgeons, this program includes 6 steps:

• Getting the right diagnosis    
• Learning about hospitals
• Finding the best doctor    
• Preparing for surgery
• Making the right treatment decision    
• Recovering at home

How your employees & their dependents can earn the Meijer gift card reward

• Members can log in to the Member Center and follow the links to Welvie. Or they can create an account using the member number from their Priority Health ID card.
• Members must complete the information, go through the first 3 steps of the surgery decision-support program (may take about 45 minutes) and do the short feedback survey.
  When they're done, a Meijer gift card reward will be mailed to them. Allow 60–90 days for processing. Rewards are limited to one per member per year.
  
• We encourage you to include this good news in your internal communications. We'll be announcing the program to eligible members on this website, in our newsletters and with targeted direct mail. Physicians in our network will also have information about the program to share with their Priority Health patients.
