Coverage documents for members of group Priority Health Medicare plans

These plans are provided by employers. They differ from plans you buy for yourself.

Go to plan information and documents for individual plan members.

• Your Summary of Benefits will be specific to the employer offering your plan.
• Your Evidence of Coverage booklet, which contains all the details of your coverage, will be specific to your employer group.
  
• Your plan year may not run from January 1 to December 31.
• If you need replacements for these documents or have questions about your coverage, call our Medicare plan customer service representatives.

Use the Priority Health Medicare plan provider network

Group plan members use the same provider network as our individual Priority Health Medicare plans.

• Check the online Find a Doctor directory for the most up-to-date information on which physicians, hospitals, pharmacies and more are in the Priority Health Medicare plan network.
• Print a copy of the 2010 Priority Health Medicare provider directory (1MB PDF)*
• Print the Member Claim Form (126KB PDF) to file a claim if you need Priority Health to reimburse you.

You use the same pharmacy network as our individual Priority Health Medicare plans.
