Simple statements

When you go to a doctor or other health care provider, we'll send you a statement like this (we call it an Explanation of Benefits, or "EOB"), showing the cost and who is paying for what.

This "header" notes the date you received care, your health care provider's name and where you received your care. You'll also see your Priority Health contract number (which appears on your ID card) and the amount of the claim that you're responsible for paying.
Here you'll see what services your health care provider charged us for and how payments were applied. It may include more than one service on the same day.

It's NOT what you owe, because you may already have paid some or all of this amount as a copay! It's the total of your share of the cost after we've paid our portion. If any of this amount is applied to your deductible or coinsurance (as outlined in your coverage documents), or if your plan specifies that you pay a copay, it's noted here. If you owe your health care provider money, you'll receive a bill from them.
