HealthbyChoice Incentives

HealthbyChoice Incentives is a health plan combined with a wellness program. It encourages you to make healthier lifestyle choices in exchange for the better of two benefit levels (Choice versus Standard). Both benefit levels offer the same coverage, but the Choice level saves you money on copayments, deductibles/out-of-pocket costs and coinsurance.

Employers also save money. The dual-benefit design offers a "combined" rate that is less than the rate would be for plans with the better level of benefits. Along with its other cost-saving features, HealthbyChoice Incentives enables employers to save up to 13 percent off the cost of health care premiums while offering employees great health care benefits.
