How to Report Fraud and Abuse

If you suspect someone of fraud or abuse, please help us keep costs down. Contact Priority Health immediately; you don't have to give your name.

Information we'll ask you for
  • The name of the Priority Health member involved in the fraud or abuse
  • The Priority Health member's card number (if possible)
  • The name of the doctor, hospital, or other health care provider
  • The date of service you suspect is fraudulent or abusive
  • The amount of money that Priority Health paid (if possible)
  • A description of the acts that you suspect involve fraud and abuse
  • Your name and telephone number (optional)
